| Job Information |
The Pathways Counselor works in a collaborative manner to build a partnering relationship with families and others in the home and community to assure that the needs of designated youth and their families are respectfully and adequately addressed.
In each case, the Pathways Counselor will assure that services to youth and families are individualized, strengths and needs based, culturally sensitive, responsive, and goal driven. This position requires local travel and flexibility to work a nontraditional schedule in a high-demand work situation.

| Job Requirements |
1. Assessment
To assure proper assessment the Pathways Counselor will:
a. gather and understand current and past social, medical, educational, psychological, and work histories
b. use diagnostic tools to provide comprehensive information for a service plan
c. assess risk factors related to safety and well being of all family members
d. provide clinical direction that meet the needs of the youth and family
2. Treatment Plan/ISP development
To assure the development of effective individual service plans/services, the Pathways Counselor will:
a. plan and hold discussions with the County or International agency for the purpose of creating service plans/ISP’s
b. work with family members to develop ownership for the service plan/ISP
c. write and utilize specific measurable goals, objectives, and time frames for each child and family
d. develop safety plan for each child and family
3. Treatment Plan/ISP implementation
To assure the implementation and evaluation of effective service plans that foster goal attainment, the Pathways Counselor will:
a. deliver services outlined in service plan/ISP
b. monitor and record client/family progress
c. provide advocacy for family to outside agencies
d. evaluate and reform goals and objectives as necessary
e. provide support to families in times of crisis,
f. complete written documents and services contracted for through private, County or International agency referrals
REQUIREMENTS:
1. Education/Credentials/License:
a. Bachelor’s Degree in human services or related field.
b. Masters Degree, preferred.
c. Valid State Drivers License and auto insurance.
2. Experience: At least one year of supervised experience with children and/or families.
3. Clearances: State Police/FBI per state regulations; Child Protection
